ABSTRACT:

Scopo: Lo scopo di questo articolo è valutare, tramite una revisione della letteratura scientifica internazionale, lo stato dell’arte dell’Eccentric Decline Single Leg Squat descritto come uno strumento di riabilitazione e prevenzione per atleti con tendinopatia rotulea.

Criteri e Metodi: Gli autori hanno effettuato una revisione della letteratura utilizzando Clinical Queries scegliendo una “Broad Strategy” e utilizzando anche MeSH Terms combinati ad operatori Booleani “AND” e “OR” e “textwords”. Sono stati esplorati come database PubMed e PubMed Central per identificare gli articoli che soddisfacevano i criteri di inclusione. Gli studi inclusi sono stati valutati sulla qualità metodologica e i dati sono stati riassunti. Sono stati inclusi 41 articoli ritenuti validi allo scopo dello studio sulla gestione clinica e terapeutica, delle tendinopatie del tendine rotuleo (Jumper’s Knee – JK) e validati in relazione alla applicabilità, sicurezza ed efficacia dell’esercizio eccentrico .

Risultati: Tutti gli studi inclusi hanno evidenziato che training eccentrici ed in particolare Eccentric Decline Single Leg Squat a 25° svolgono un ruolo importante nel trattamento, conservativo, della tendinopatia rotulea. Anche se i risultati, dopo 12 settimane, devono essere interpretati e studiati singolarmente, l’effetto del trattamento potrebbe essere stimato con un miglioramento significativo della funzionalità del ginocchio e del dolore, (range di riferimento 60-80% dei pazienti) Conclusioni Possiamo affermare che nella pratica clinica riabilitativa l’ Eccentric Decline Single Leg Squats a 25° risulta essere uno strumento altamente efficace e modulabile nel management del ritorno allo sport dell’atleta con Tendinopatia Rotulea (TR) L’esercizio eccentrico su piano declinato in relazione alla letteratura da noi scansita, decrementa il dolore, migliora la funzione articolare e le abilità tecnico atletiche specifiche ma, dobbiamo sottolineare, come i tempi (12 settimane di trattamento eccentrico) per il ritorno alla piena attività agonistica evidenziati dalla letteratura ,rimangono, secondo noi, obiettivamente lunghi.
